Hospice Case ManagerA minimum of 2 years' experience in Hospice/Home Health is required.
Experience in both areas preferred.
Seeing patients in their homes.
40 hours per week, 8 hour day shifts, rotating on call evenings and weekends.
Overtime will be paid for any hours worked over 40 in a Sun-Sat work week.
May be asked to assist with Home Health.

Shift: 5 eight hour dayshifts with call.
