**Transitional Care Registered Nurse**The Job: Provide transitional care nursing support to Clinical Improvement Department. Priorities are activities that promote continuous compliance and improvement activities to meet Medicare and Medicaid Conditions of Participation, Joint Commission Accreditation Standards, and others as needed to obtain or maintain hospital accreditation or certifications.Colorado West HealthCare System participates in Centers for Medicare and Medicaid (CMS), and is deemed in compliance with those Conditions of Participation through its successful accredited status from the Joint Commission. CWHS is committed to the provision of quality health care to the community it serves, and is accountable to collect and submit data from which the organization is measured. Further, CWHS is committed to a continuous monitoring of the quality of care provided, and collects, analyzes, and presents data to the board and medical staff, providing actionable information from which to assess clinical competence and quality outcomes.Work Schedule: Full-TimeLocation: Community HospitalResponsibilities:Collaborates with the interdisciplinary team to thoroughly plan for safe discharge of complex patients.Discharge teaching, including disease teaching, self-care, lifestyle changes, needed medical equipment (such as oxygen, incentive spirometry) and medication reconciliation/education. Schedules primary care provider (PCP) appointment for patient before discharge. If the patient has no PCP, the TCRN works with case management to arrange follow-up, or helps patient to fill out application for PCP.Requirements:Bachelor's Degree in NursingFive (5) years of healthcare organization experience. Current working knowledge of Rules/Regulations CMS (CoPs), ORYX Core Measures, and accreditation bodies.TJC/DNV preferredThis position's pay range is: $26.43-37.00 per hour, depending on education and experience.Discretionary bonuses, relocation expenses, merit increases, market adjustments, recognition bonuses and other forms of discretionary compensation may be paid to eligible employees based upon organizational and individual performance.Benefits:Employees are eligible to participate in an attractive benefits package including medical, dental, vision, paid time off, education assistance, 403(b) with employer matching, and more.
