POSITION OVERVIEW: Primary focus is on preventative care, promoting the health and well-being of the clinic's patient population. Provide services to help patients stay healthy, avoid or delay the onset of disease and lead a productive healthy life. Responsible for providing professional care for clinic patients following established standards and practices. Assists providers and staff in coordinating patient care. Functions as the primary liaison between patients, staff and providers. Annual Wellness employee will be cross trained with the Care Manager. Receives direction from: Providers, Care Manager and Clinical Managers/Supervisors.SHIFT: Full Time, Monday - Friday, 8:00AM - 5:00PMMINIMUM QUALIFICATIONS: Graduate from nursing program or certified medical assistance program. LICENSE/CERTIFICATIONMontana Nursing licensure or Certified Medical Assistance certificate. Basic Life Support. KNOWLEDGE/SKILLS/ABILITIES Medical Terminology. Basic Nursing Principles and Procedures. Basic computer and key boarding skills. Core values consistent with patients and family centered approach of care. Demonstrates professional, appropriate, effective, and tactful communication skills including written, verbal and nonverbal. Must be detail oriented and have good assessment skills in order to determine the needs of the patients
