CNA Outpatient Hospice Please only submit candidates that area aware of their submission to our facility.

Traveler must have permanent address outside of radius rule of 75 miles.
Please note time off requests must be disclosed with initial submission to be considered for approval.
Time off requested after the acceptance date will not be approved.
Summary Provides skilled aide care to the patient under the supervision and direction of the primary registered nurse or clinician and under an aide care plan as ordered in conjunction with the physician.
13 weeks assignment, 2 years' experience current required in Outpatient Hospice.
Must be a team player, flexible and accustomed to fast paced environment.
Must have successfully completed at least one 13-week travel assignment.
BLS certification required.
BLS must be completed through American Heart Association, American Health and Safety, or American Red Cross.
When submitting candidates include at least two references (one must be from current manager), a resume, and completed skills checklist/competency assessment.
Shifts may fluctuate to fulfill department staffing needs.
On call, weekends and holidays may be required.
Travelers are first to float within facility and to alternate locations within 20 miles of permanent assigned location.
Traveling pairs are not guaranteed same schedules or same shifts.
Orientation is the first week of the contract, not to exceed 40 total hours, and includes EPIC training.
Scrubs required and provided by the facility.
This is a 8 hour Day shift position.
