JOB DESCRIPTION:

The Acute Care Unit is located on the second floor of Geisinger Jersey Shore Hospital, in Jersey Shore, PA.
It is comprised of two wings with a central nursing station.
There are sixteen (16) rooms available to accommodate a maximum of 21 patients, eleven of those rooms are private rooms; the other five rooms are semi private.
There are eight telemetry monitors available for use throughout the second floor.
Other private rooms are available within the unit per patient condition and patient request (as census permits) and, if necessary, for infection control practices.
The Acute Care Unit is designated for medical-surgical, observation bed, 23-hour stay patients, and PCU patients.
Swing bed patients (similar to SNF) are also included in the mix of patients.
The Acute Care Unit is designed to care for medical and surgical patients of all ages including those who require cardiac monitoring.
Exceptions to these services include: 1.
Neonates 2.
Invasive hemodynamic monitoring 3.
Mechanical ventilation 4.
Titrated cardiac meds Professional Registered Nurse 1.
Management of patient care in the Medical Surgical Telemetry Unit according to the nursing process to include assessment, planning, implementation and evaluation.
Actively participates in the promotion of quality patient care.

Leadership abilities are demonstrated in functioning as a professional role model with consumer relations and communications skills, problem solving and decision making and assistance in orienting and training personnel.
3.
Collaborates with other healthcare providers to coordinate the patient's plan of care.
4.
Include patient in care by participating in discharge planning, update patient communication board, handout daily newsletter, perform hourly rounding to asses patient's needs, and include patient and family in teaching.
5.
Focus on Teamwork and a multi-disciplinary team approach, while showing compassion and empathy to the community we serve.
Nurse to Patient ratio is 4-5:1 on all shifts.
If nurse is assigned an LPN, ratio could go to 6-7.
NA/UC are also a part of the staffing model.
Assessment are to performed on patients once during the nurses' 8-12 hour shift or with condition change.
Swing bed patients only require a daily assessment.
Certification Requirement: BLS upon hire, ACLS and PALS within 6 months of start date.
Our leadership is compromised of 1 full time direction and a PRN clinical assistant.
