As part of a multi-disciplinary team, The Nurse Care Manager will serve as vital role on a harm reduction team for patients enrolled in the Recovery Services, an Office-Based Opioid Treatment (OBOT) program.
Additionally, nurse care managers are responsible for on-going coordination of follow-up care, close telephone monitoring, relapse prevention, and support for patient self-management.
Monday-Friday, 8:00am-5:00pm with evening rotations.

Prioritized Must Have Skills for the Registered Nurse:

#1.
Nursing Degree required (ASN or BSN)

#2.
BLS/CPR Card required.

#3.
MA RN License required.

#4.
No more than 3 jobs in the past 10 years.

Responsibilities of the Registered Nurse:

  • Completes initial patient assessment and intake obtaining social, medical, mental health, substance use and medication history.
  • Collaborates and communicates with providers in establishing an appropriate plan of care and revising as appropriate.
  • Meets regularly with addiction treatment team on a weekly, scheduled basis, along with regularly communicating with team as needed patient needs.
  • Reviews and obtains signatures for program consents and treatment contracts with patients.
    Provides patient with program handbook.
  • Provides the assessment of signs/symptoms of opioid withdrawal along with assessment and monitoring of patients in the induction, stabilization, and maintenance phases of treatment.
  • Provides ongoing education, assessment, management, and support of patients in all phases of treatment including induction, stabilization, and maintenance phases of treatment.
  • Completes the ordering and reviewing of urine toxicology screens, routine labs under appropriate prescriber.
  • Follows State and Federal guidelines in providing care to patients in collaboration with licensed, prescribing physicians and pharmacy.
  • Provides ongoing assessment of medical care needs in conjunction with substance use support needs using appropriate assessment techniques.
  • Prioritizes collected information provided by patient according to the patient's immediate conditions and needs.
  • Communicates oral/written information clearly and effectively when sharing/documenting relevant patient information; documentation complying with departmental policies.
  • Provides follow up care in person and via telephone when needed.
  • Perform other duties as specified by the nurse manager and program director.
  • Requirements of the Registered Nurse:

    • Bi-lingual Spanish speaking skills preferred.
    • Nursing Degree required (ASN or BSN)
    • BLS/CPR Card required.
    • MA RN License required.

    Other Key Requirements:

    • 100% onsite, in-office role.
    • No sponsorships or visa holders.
      No Corp-to-Corp.
