Position Summary: Min 2 years exp in acute care setting or rehab setting required. The Certified Nurse Assistant/Patient Care Technician provides direct and indirect basic health care to patients ranging in age from 18 years to geriatric. Assists as needed with environmental and informational services; and communicates effectively to ensure positive outcomes for patients. They are responsible for performing certain procedures according to hospital policy and procedures. Promotes the mission and values of the hospital. Demonstrates the ability to maintain an understanding attitude and responsible concern toward patients which is evidenced in speech and actions, and shows interest/ability to acquire knowledge of and develop skills in basic nursing procedures. Position Supervisory Responsibilities: None Position Qualifications: Minimum Qualifications: Ability to project a professional image Knowledge of regulatory standards and compliance requirements Strong organizational skills Working knowledge of computer and software applications used in job functions (Order entry, data input) Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in this position Freedom from illegal use of drugs Freedom from use of and effects of use of drugs and alcohol in the workplace Education and/or Experience: High school diploma or equivalent preferred CNA/PCT certificate for state of employment Certificates, Licenses, Registrations: Current state certificate to practice as a CNA/PCT Must maintain current provider CPR certification throughout employment Language Skills: English is the primary language of the facility Ability to chart patient information correctly Ability to effectively present information and respond to questions from supervisors, patients, families, and physicians Mathematical Skills: Ability to add and subtract using whole numbers, common fractions and decimals Ability to identify percent of meal consumed Reasoning Ability: Ability to carry out written and oral instructions Ability to prioritize tasks and perform essential tasks first Asks for assistance or clarification when needed Essential Functions and Responsibilities To perform this job successfully, an individual must be able to perform each key function satisfactorily based on five overall skills categories; assessment, planning, intervention, evaluation and teaching/coaching Each category will be assessed on performance measurements of appropriateness, efficiency, effectiveness and timeliness Reasonable accommodations may be made to enable individuals with disabilities to perform the key functions Job Function Approximate % of Time Demonstrates ability to recognize patient or environmental emergencies and initiate appropriate alarms (call Code Blue, Code Red, etc.). 5% Reports changes in patient status and documents clinical procedures appropriately. Maintains patient confidentiality. 5% Performs basic monitoring of patients (i.e. vital signs, height, weight, intake and output) and collects specimens for diagnostic studies (stool, urine, sputum, blood glucose); documents information on graphic sheet in patient medical record in a timely manner. 10% Supports individualized plan of care by assisting patient with activities of daily living, personal care, nutritional/ fluid needs, elimination, ambulation and exercise. 20% Assists with admissions, discharges, transports and transfers of patients using proper body mechanics and proper safety procedures. 25% Performs basic procedures to include, but not limited to, assisting patients with elimination needs, skin care and oral care. Light housekeeping duties may also be required. 20% Assists with maintaining a clean, safe and orderly environment to include, but not be limited to designated housekeeping tasks, maintenance of equipment and stocking supplies. 5% Demonstrates knowledge and skill necessary to care for ranging in age from 18 years to geriatric, particularly geriatric, on assigned units. 5% Maintains required certifications, JCAHO and other regulatory requirements and department specific training/competencies and mandatory updates. Participates in meetings/committees as requested. 2% Performs other duties as assigned. 3% Additional Responsibilities: Ability to carry out the essential functions of this position (with or without reasonable accommodation). Attends and participates in facility meetings as required Attends continuing education opportunities appropriate to responsibilities Complies with HIPAA regulatory requirements Demonstrates ability to prioritize tasks/responsibilities and complete duties within allotted time Demonstrates and understands the importance of and respect for the rights, dignity, and individuality of each patient in all interactions Demonstrates honesty and integrity at all times in the care and use of patient and hospital property Demonstrates respect for co-workers and responds to needs of patients by complying with hospital policies Demonstrates satisfactory level of interpersonal skills to interact with facility staff, Administration, patients, families, customers, vendors, government and regulatory agencies Knowledge of safety, emergency and disaster procedures of the facility Participates in hospital wide patient safety program identifying risks to patient safety and reducing healthcare errors Participates in relevant in-service training sessions Participates in hospital survey readiness programs Promotes the philosophy, mission and objectives of the Company within and to customers outside the hospital Performs other duties as assigned Patient Rights Promotes and protects patients' rights; treats patients with dignity and respect; reports suspected abuse or neglect. Leadership Demonstrates willingness to try new tasks; generates new ideas for change; evaluates and recognizes priorities; communicates and models organizational values; fosters high performance; recognizes need for and provides adequate resources. Process Improvement Applies process improvement methods and techniques; assists in data collection; identifies processes for improvement in daily work; and educates new employees in team process. Environment of Care Demonstrates understanding of fire and emergency procedures; participates in fire and disaster drills; demonstrates understanding of safety and security procedures; applies safety and security precautions; demonstrates understanding of hazardous materials plan; demonstrates proper use of equipment. Infection Control Applies hand-washing principles during daily work; demonstrates understanding of isolation precautions; recognizes signs and symptoms of infection and complies with the employee health program; demonstrates understanding of the process for identifying and handling infectious waste; maintains personal hygiene; complies with OSHA standards in the work place; and demonstrates understanding of cross-contamination. Information Management Enters or records data timely and accurately; protects confidentiality of patient information; protects data against loss or destruction; reports suspected violation of security/confidentiality issues.
