Armina Health is looking for a capable Manager to coordinate a team focused on providing the full continuum of care throughout the patient's journey. This role is a chance to make a real difference in our patients' care journey. Armina Health is a healthcare company that values its team, offering competitive pay and benefits. As a member of Armina Health’s team, the Manager, Care Coordinator will be responsible for supporting all satellite clinics with the full continuum of care throughout the patient's journey, working closely with healthcare providers, referral partners, and other stakeholders to ensure timely and efficient care delivery. In addition to coordinating patient care, the Manager, Care Coordinator will also be responsible for immediate support and supervision of the Lead Care Coordinator. 

The Care Coordination Manager will manage multiple aspects associated with the care coordination process for Armina Health. This position will build and maintain a system for coordinating patient care from the first visit to the last visit. 

DUTIES & RESPONSIBILITIES

·      Review patients managed by care coordination team 

·      Track and monitor all potential new patients’  daily to distribute to the Physician Liaisons and Practice Managers weekly

·      Manage escalated issue resolution by phone and/or in person 

·      Communicate issue resolutions to Practice Managers, Physician Liaisons, HR, and Compliance as needed

·      Manage answering service; in-office/after-hours answering service Ensure all ethical and legal obligations are met

·      Manage the in-office communications for the company including phone, fax, and email

·      Manage patient transportation as directed 

·      Manage time and attendance system 

·      Additional projects, as assigned.

  •  Experience in patient care coordination or a related role
  •  Strong business and sales background
  • 2+ years call center management experience
  • 2+ years Personal Injury experience
  • Must have strong commitment to client service
  • Previous management experience
  • Excellent writing skills
  • Working on cross-functional teams
  • Previous customer service/sales experience preferred with after-hours on call experience
