Now Hiring - Advanced Practice Nurse - Southwest NJ Team PRN Advanced Practice Nurse The Advanced Practice Nurse will be responsible for, but are not limited to: Conduct re-certification visits to patients on hospice in accordance with local, state and federal regulations. Communicate visit findings to designated members of the interdisciplinary team, including the Medical Director. Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment). Consult with the referring physician, the acute care team, and other members of interdisciplinary team to contribute to the patient's plan of care. Prepare and maintain accurate patient records, charts and documents. Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed. Ensure collaboration with patients, caregivers, medical directors and members of IDT. Education Master's Degree in Nursing Professional Experience Must have a Nurse Practitioner's license. Must have 2-4 years experience working within hospice or related field. Must have a Master's Degree. Must have a valid driver's license and a reliable source of transportation.
