Advanced Practice Nurse Apply
SUMMARY We are seeking a dedicated Advanced Practice Nurse to provide high-quality direct patient care, supervised by physician practitioners, within our primary care organization. This role involves being part of an on-call rotation, focusing on geriatric patients in skilled nursing centers. ESSENTIAL DUTIES AND RESPONSIBILITIES Manage a defined caseload of geriatric patients in skilled nursing settings. Conduct timely patient visits to the care center or other designated locations. Issue appropriate medical orders and maintain thorough documentation and assessments. Engage with patients and/or their families in a compassionate and timely manner. Demonstrate a commitment to patient service, community engagement, and the mission and vision of our organization. Facilitate positive relationships among medical staff, nursing, patients, and families. Build collegial relationships with physicians and fellow practitioners. Participate in physician-led initiatives focusing on evidence-based, patient-centered care, quality assurance measures, and clinical guidelines. Maintain effective communication across departments and organizations to foster collaboration. Participate in organization-wide surveys and mandatory reporting as required. Uphold patients' rights to fair, equitable treatment and privacy. Comply with our Corporate Compliance Program. Report any job-related functions that involve occupational hazards, including exposure to blood and body fluids. Enhance the organization's reputation through servant leadership and open communication. Contribute to committees, workgroups, project management, and collaborative efforts. Perform additional duties as needed to ensure the success of our organization. SKILLS AND ABILITIES In-depth knowledge of medical care regulations and payment processes. Awareness of organizational procedural guidelines and compliance. MINIMUM QUALIFICATIONS Valid Registered Nurse license with authorization for advanced nursing practice in accordance with Georgia Board of Nursing requirements.
