???? About the Role

PanCare of Florida is seeking a skilled, compassionate Advanced Practice Registered Nurse (APRN) to join our primary care team. In this role, you will provide comprehensive, patient-centered healthcare services to individuals and families across the lifespan, managing both acute and chronic conditions within your scope of practice.

As part of a mission-driven, community-based healthcare organization, you’ll play a vital role in improving access to high-quality medical care for underserved populations. What You’ll Do

In this role, you will:

  • Conduct comprehensive patient assessments, including medical history, physical exams, and differential diagnoses
  • Diagnose and treat acute and chronic conditions such as diabetes, hypertension, asthma, and common infections
  • Order, perform, and interpret diagnostic tests and procedures
  • Develop, implement, and manage individualized, patient-centered care plans
  • Prescribe medications, therapies, and lifestyle modifications with appropriate documentation and education
  • Provide preventive care, health screenings, and wellness services
  • Educate patients on disease prevention, health promotion, and chronic condition self-management
  • Maintain accurate, timely documentation within the Electronic Health Record (EHR)
  • Collaborate closely with physicians, nurses, and interdisciplinary care teams
  • Coordinate referrals, follow-ups, and continuity of care
  • Support quality improvement initiatives, clinical protocols, and compliance efforts
  • Mentor clinical staff and contribute to a positive, team-oriented work environment

  • Excellent interpersonal, communication, and patient education abilities
  • Ability to manage complex cases and prioritize care effectively
  • Cultural sensitivity and awareness of social determinants of health
  • Strong EHR and clinical documentation skills (eClinicalWorks preferred)
  • Knowledge of HIPAA, healthcare regulations, and ethical standards
  • Leadership and mentorship capabilities within a multidisciplinary team

Education & Experience

  • Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
  • Completion of an accredited APRN program (Family Practice or General Practice preferred)
  • National certification (e.g., FNP through AANP or ANCC)
  • Active, unrestricted Florida APRN license
  • Minimum of 2–3 years RN experience; 1–2 years in primary care or general practice preferred
  • Experience managing common primary care and chronic conditions
  • BLS certification required
  • Specialty certifications (Women’s Health, Pediatrics, Gerontology) a plus
  • Experience with EHR systems, especially eClinicalWorks, preferred

️ Physical Requirements

This role requires:

  • Frequent standing and walking during clinic operations
  • Ability to lift up to 50 lbs. and assist with patient mobility as needed
  • Manual dexterity for examinations, procedures, and documentation
  • Ability to bend, stoop, reach, and move equipment or supplies
  • Visual and auditory acuity for patient assessment and diagnosis
  • Emotional resilience to manage high-volume clinics and complex patient needs
