Job Description
As a Family Nurse Practitioner (APRN) with Healthy Connections, you will deliver integrated, high-quality care to patients of all ages. This role is ideal for a provider who values teamwork, evidence-based medicine, and a holistic approach to health — including support for patients receiving Medication Assisted Treatment (MAT).
You will work closely with a collaborative team of physicians, behavioral health professionals, and support staff to ensure patients receive comprehensive, judgment-free care.
Key Responsibilities
- Perform physical exams, patient assessments, and detailed medical, family, and social histories.
- Diagnose and manage acute and chronic illnesses in a primary care setting.
- Provide patient-centered Medication Assisted Treatment (MAT) services and support adherence to treatment plans.
- Order, interpret, and follow up on diagnostic and laboratory tests.
- Prescribe medications and treatments as appropriate.
- Educate and counsel patients and families on health conditions, treatment options, and lifestyle changes.
- Coordinate care with supervising physicians, specialists, and other team members.
- Participate in quality improvement initiatives, including Patient-Centered Medical Home (PCMH) and Meaningful Use (MU) goals.
Qualifications
- Graduation from an accredited Nurse Practitioner program.
- Current Arkansas APRN license and DEA registration.
- Experience providing Medication Assisted Treatment (MAT) preferred.
- Basic Life Support (BLS) certification.
- Excellent communication, organizational, and critical thinking skills.
- Commitment to patient-centered, community-based healthcare.
- Proficiency with Microsoft Office and electronic health record systems.
