Key Responsibilities

• Collaborate with the team to provide management and delivery of patient care plans

• Support patients in South Park and surrounding areas

• Consistently meet critical charting and documentation deadlines

• Self-manage time and scheduling effectively

• Utilize tablets, mobile phones, and EMR software proficiently

Schedule and Shift Details

• Monday through Thursday, 5:00 p.m. to 8:00 a.m.

• May consider Monday through Friday, 5:00 p.m. to 1:30 a.m. schedule

Travel

• Local travel required within the service area

Qualifications

Required

• Associate degree or higher in Nursing

• Minimum of 2 years hospice nursing experience

• At least 1 year experience with Electronic Medical Records (EMR), including TruChart EHR or other systems

• Valid Registered Nurse (RN) license in Pennsylvania

• Valid Driver's License, reliable transportation, and auto insurance

• Comfortable with the travel requirements for this position

Preferred

• RN license number available upon application
