Position Title Ambulatory Clinic Nurse Care Coordinator (RN) – Spine Center, Days – Full Time, Sutherland Institute Job Summary The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence‑based, safe, and compassionate nursing care. The Coordinator focuses on patient safety and the quality of nursing care by identifying and clarifying patient needs, performing clinic‑related patient care, conducting health education, promoting patient advocacy, coordinating health‑care services, and evaluating patient outcomes. The RN Care Coordinator will support assigned physicians by communicating with patients, families, and caregivers, ensuring that all orders placed during clinic sessions are completed, and that physicians are notified of results. Responsibilities Manage patient flow, triage, and direct patient care. Act as liaison among front desk staff, nursing staff, physicians, and clinic manager. Develop expected patient outcomes, define appropriate resource utilization, and work with providers to set patient goals. Provide nursing support and coordination to optimize patient outcomes and improve patients’ quality of life through clinical and educational programs and/or support services. Support patient self‑management of disease. Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Manage high‑risk patient care, including patients with multiple comorbidities or a high risk for readmission, and maintain a registry. Establish relationships with patients and use assessment skills to identify physical, psychosocial, financial, and environmental health‑care needs. Educate patients and ancillary personnel about health‑care needs, safety issues, and benefits, including patient education. Utilize electronic tools, such as EMR, to document assessments, clinical findings, treatment plans, and progress toward goals. Observe, record, and report patient conditions and reactions to drugs and treatments to physicians. Educate patients and families about diagnostic procedures, medications, nutrition, and maintenance of health and wellness. Maintain and review patient records, charts, and other pertinent information. Record and follow up on test and examination results; post results as needed. Respond to or refer incoming patient‑treatment related phone calls. Instruct patients and families regarding medications and treatment instructions. Maintain patient confidentiality at all times. Document patient care using EMR to maintain and develop patient records in compliance with practice standards. Perform all professional, clinical, and/or technical competencies required by the assigned unit or department. Required Qualifications Education : High‑school graduation, bachelor’s degree in nursing, and at least two (2) years of RN experience. Licensure : Licensed Registered Nurse (LRN) – Multi‑State – State Board of Nursing; current active multi‑state RN license in good standing in the state of residence. Staff hired prior to 2/1/2024 must obtain a multi‑state RN license from the state of residence at the time of their next renewal by 4/30/2026. Certification : Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA) within 14 days. Experience : Two (2) or more years of RN experience in a clinical setting. Preferred Qualifications Previous medical office experience. Knowledge and Skills Basic typing and word‑processing ability. Excellent analytical, critical‑thinking, and problem‑solving skills.
