Position Title Ambulatory Clinic Nurse Care Coordinator (RN) – Neurology Days – Full Time – Landon Center on Aging
Position Summary The Ambulatory Clinic RN Care Coordinator is a licensed registered professional nurse who is a clinical expert providing ethical, evidence-based, safe, and compassionate nursing care. The RN focuses on patient safety and quality of nursing care by identifying and clarifying patient needs, performing clinic‑related patient care, conducting health education, promoting patient advocacy, coordinating healthcare services, and evaluating patient outcomes. The RN Care Coordinator supports physicians by communicating with patients, families, and caregivers and relaying information to the physician. The RN provides nursing care services during physician clinic sessions, verifies orders, ensures patients and families understand the plan of care, and communicates results to the physician.
Responsibilities & Essential Job Functions Manage patient flow, triage, and direct patient care.
Liaise communication between front desk staff, nursing staff, physicians, and clinic manager.
Develop expected patient outcomes, define resource utilization, and set patient goals with the provider.
Provide nursing support/coordination to optimize patient outcomes and improve quality of life through clinical and educational programs and/or support services.
Support patient self‑management of disease.
Coordinate continuity of care with patients and families after hospital admission, discharge, and ER visits.
Manage high‑risk patient care, including patients with multiple comorbidities or high risk for readmission, including registry management.
Develop relationships with patients and use assessment skills to identify physical, psychosocial, financial, and environmental health‑care needs.
Educate patients and ancillary personnel about health‑care needs, safety issues, and benefits, including patient education.
Utilize electronic tools such as EMR to document assessments, clinical findings, plans of treatment, and progress toward goals.
Observe, record, and report patient’s condition and reaction to drugs and treatments to physicians.
Educate patient/family about diagnostic procedures, medications, nutrition, and maintenance of health and wellness.
Maintain and review patient records, charts, and other pertinent information.
Record tests and examination results, post results, and follow up with patients.
Respond to and refer incoming patient treatment‑related phone calls.
Instruct patient and family regarding medications and treatment instructions.
Maintain patient confidentiality and document using EMR in compliance with practice standards.
Perform professional, clinical, and/or technical competencies of the assigned unit or department.
Required Education and Experience High School Graduate; Bachelor Degree in Nursing; 2 or more years of RN experience.
Preferred Education and Experience Previous medical office experience.
Required Licensure and Certification Licensed Registered Nurse (LRN) – Multi‑State – State Board of Nursing. Must have current active multi‑state RN license in good standing in the state where you live.
Staff hired prior to 2/1/2024 must obtain a multi‑state RN license by 4/30/2026.
Basic Cardiac Life Support (BLS or BCLS) – American Heart Association (AHA) within 14 days.
Knowledge Requirements Basic typing and word processing ability.
Excellent analytical, critical thinking, and problem‑solving skills.
