As an independent contractor, you will play a crucial role in assessing the needs of our clients and developing care plans to ensure their well-being. This is a per visit pay job.Responsibilities:Conduct comprehensive assessments of clients' physical, emotional, and mental healthDevelop individualized care plans based on assessment findingsCoordinate with other healthcare professionals to ensure holistic care for clientsProvide education and support to clients and their familiesDocument assessment findings and care plans accurately and timelyRequirements:Valid RN license in the state of IllinoisMinimum of 2 years of experience in home health or a similar settingStrong communication and interpersonal skillsAbility to work independently and collaboratively within a teamCompassionate and empathetic demeanorAbout Us:HomeWell Care Services IL250 has been providing exceptional home care services in Glendale Heights, IL for over 10 years.
