Backfill Contract Rn Case Management Weeks Apply
Position: RN - Case Management
Location: El Paso, TX 79902
Pay Scale: $2,100 per week
Shift: Days, 7:30 AM - 4:00 PM
Responsibilities:
- Coordinate patient care across the healthcare continuum, ensuring efficient and effective transitions.
- Perform bedside case management, providing support and advocacy for patients and their families.
- Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized care plans.
- Utilize Meditech for documentation and case tracking.
- Apply knowledge of post-acute care to ensure appropriate follow-up and patient outcomes.
Requirements:
- Active RN License: TX or Compact.
- Minimum of 1 year of bedside case management experience.
- Familiarity with HCA systems and processes.
- Knowledge of post-acute care practices.
- Basic Life Support (BLS) certification.
