Job Summary: As a part of Choptank Community Health (CCHS) multidisciplinary team, the Behavioral Health Clinician will provide high quality, patient-centered behavioral health services, in accordance with the policies and procedures and values of CCHS. This role delivers both integrated, brief interventions as part of the care team and traditional outpatient therapy for CCHS patients requiring longer-term treatment. These direct services may include care for individuals (children and adults), couples, families, and group therapeutic services to patients. Working collaboratively with medical, psychiatric, and the population health team, the clinician conducts comprehensive assessments, develops evidence-based treatment plans, and offers culturally responsive counseling to diverse populations. Responsibilities include crisis intervention, care coordination, and participation in multidisciplinary team meetings to support whole-person care. The clinician promotes accessible, trauma-informed behavioral health services designed to improve patient functioning, address social determinants of health, and enhance overall wellness within the community. This is an exempt, full-time position. Licensure, credentials, and experience will determine pay within the pay range of $76,843.00 - $115,264.00. (Salary will be determined based on licensure and experience.) The Behavioral Health Clinician reports directly to the Behavioral Health Director or designee and works closely with the Clinical Supervisor and Behavioral Health Clinician Lead.

Required Skills/Abilities:

  • Working knowledge of MD Mental Health policies and procedures.
  • Knowledge and experience with evidence-based practices, including trauma informed care, substance use treatment, and/or co-occurring disorders.
  • Strong clerical/administrative skills including computer skills. Electronic Health Record knowledge preferred.
  • Ability to work effectively in a fast-paced, team-based healthcare environment
  • Excellent communication and interpersonal skills
  • Cultural humility and commitment to health equity
  • CPR Certified within orientation period.
  • Working knowledge of ICD and CPT coding.
  • Valid MD driver’s license

Education and Experience:

  • Current state licensure (LCSW-C, LCPC, LMSW, LGPC).
  • Graduate of an accredited university Master’s degree program in a related Behavioral Health field
  • Must be able to meet and maintain the CCHS credentialing and privileging requirements
  • Experience providing behavioral health services in healthcare, community mental health, or integrated care settings to children, adults, and families.

Working Conditions and Physical Requirements:

  • General office environment in health centers or schools (if applicable), may include telehealth
  • May involve collaboration across multiple clinic sites
  • Occasional travel

Duties/Responsibilities:

  • Clinical Services
    • Provide evidence-based individual, family, and group psychotherapy to children, adolescents, and/or adults
    • Conduct comprehensive behavioral health assessments, screenings, and diagnostic evaluations utilizing measurement-based care
    • Develop, implement, and regularly update individualized treatment plans in collaboration with patients and care teams
    • Provide crisis intervention, safety planning, and short-term therapeutic support as needed
    • Address a wide range of behavioral health concerns, including depression, anxiety, trauma, stress, adjustment disorders, and substance use disorders as appropriate
    • Maintain an active caseload, based on the needs of the program, and in accordance with COMAR regulations, utilizing electronic tools to track active and archived/discharged patients.
  • Integrated Care & Collaboration

    • Collaborate with primary care providers, psychiatrists, case managers, and other healthcare professionals in an integrated care model
    • Participate in case consultations, team meetings, and care coordination activities
    • Provide behavioral health consultations within primary care settings when appropriate, including warm hand-offs
    • Educate patients and families on mental health conditions, coping strategies, and treatment options
  • Documentation & Compliance

    • Maintain timely, accurate, and compliant clinical documentation in the electronic health record (EHR), closing notes within 72 hours.
    • Ensure documentation meets FQHC, state, federal, and payer requirements (including HRSA standards)
    • Documentation includes intake paperwork, biopsychosocial assessment, diagnostic screens, and assessments as necessary, treatment planning, discharge planning, satisfaction survey, and community referrals when needed.
    • Adhere to all confidentiality, HIPAA, and professional ethical standards
    • Participate in quality improvement initiatives and outcome tracking by fully utilizing measurement-based care and staying engaged with electronic tools
  • Community & Population Health

    • Provide culturally responsive and trauma-informed care to diverse and underserved populations
    • Support patients in addressing social determinants of health through referrals and coordination with community resources
    • Assist with outreach efforts and health education initiatives as assigned
  • Professional Development

    • Maintain active licensure and meet continuing education requirements
    • Participate in clinical supervision, training, and professional development activities
    • Stay current with evidence-based practices and emerging trends in behavioral health care
    • Other duties as assigned
