JOB DESCRIPTION:

Nominated by our own employees, Corus Health is the proud recent recipient of Albuquerque's prestigious Top Workplace award. As a full-service hospice and home care agency providing a broad range of services to our customers, we are currently seeking a Full Time Physical Therapist to help our patients regain independence by providing therapy services in their home environments primarily in Rio Rancho & Bernalillo. Under the Home Health Director's supervision, our therapists perform patient assessments and implement individualized plans of care to help our clients achieve their functional goals.

Responsibilities & Duties:

  • Performing home visits to evaluate patient's level of function
  • Developing an individual plan of care based on the client's personal goals
  • Implementing the treatment program through direct treatment of the patient
  • Reassessing clinical signs and symptoms to determine effectiveness of program
  • Instructing patient, family, and interdisciplinary team members regarding participation in the treatment program
  • Ability to effectively supervise Physical Therapist Assistants as part of the interdisciplinary team

Requirements

Skills & Qualifications:

  • Degree in physical therapy from a university accredited by the American Physical Therapy Association
  • Home health experience strongly preferred
  • License to Practice by the State of New Mexico
  • Comprehensive knowledge of the principles, concepts, and methodology required of an physical therapist and its practice application in the treatment of hospice patients at home
  • Excellent Communication and documentation skills
  • Willingness to work with a interdisciplinary team
  • Valid New Mexico driver’s license and reliable, insured transportation
