Assist, coordinate & treat patients with mental/substance use disorders at a nurse-led health clinic that uses a collaborative integrated healthcare model. This position will collaborate with the Executive Director, clinical psychiatrist, psychologist, NPs and office staff as a liaison between staff, other behavioral health providers and community services to provide comprehensive treatment.
Leadership:

Ensure safe and productive operation of clinic through attentive administrative and clinical support

Ensure patients enjoy a positive experience and are treated with care and compassion
-
Complete tasks associated with patient care, uphold confidentiality requirements & manage daily activities consistent with HIPAA, federal regulations & clinic standards

Establish favorable relationships with patients, instruct patients and family members with information related to prevention and awareness of substance abuse/mental health

Establish favorable working relationships with staff including nurses, physicians and center operations staff
Duties:

Develop, implement & evaluate mental health plans with Psychiatrist, Psychologist, Medical Staff & other providers

Perform comprehensive biopsychosocial assessment for mental health, substance abuse, co-occurring disorders and medical needs & conduct follow up based on results

Address referrals including early intervention to more intensive psychological or psychiatric services as needed

Responsible for the content of education & counseling programs for individual patients and groups

Maintain list of community linkages to meet needs of target population to refer as needed

Assist patients in determining eligibility for health coverage programs and other services as needed

Participate in regular team meetings & meet with Executive Director as a means of enhancing personal growth, reviewing care management services & admin/office responsibilities

Maintain client records, including mandated agency forms with confidentiality & professionalism
Education/Skills Needed:
-
Graduate Degree & unrestricted license to practice in state of Alabama

Licensed Clinical Social Worker, Licensed Professional Counselor Licensed Marriage & Family Therapist OR Psychologist
-
Knowledge of serious mental health issues, mental health assessment, consumer rights, drug abuse & addiction
-
Scheduling, word documents & keyboard skills
-
Demonstrated success in customer service/patient services or working with the general public preferably in a medical care facility
