Will assist in the development and maintenance of a healthy and supportive environment. Will work with individuals diagnosed with mental illness, substance use disorders or needing primary care to promote recovery, independence and successful community living. Facilitate development of a Service Plan with participants and provide referrals to appropriate programs and service providers. Ensure timely documentation of services and outcomes. Requirements Adhere to all local, state and federal privacy regulations, I including HIPPA regarding confidentiality. Case management; monitor, evaluate and track clients. Collect and report client outcomes related to behavioral health services. Must be trained and certified as a peer recovery coach through a Peer within six months of hire. Must be able to obtain certification in CPR and First Aid (within 30 days of employment). Valid Oregon's Driver License and insurable by CTWS. Pass a pre-employment Alcohol and Drug test and a Criminal History check with Federal, Tribal and State Law Enforcement. Benefits Excellent medical insurance with low annual maximum-out-of-pocket costs, plus vision and dental. Available flex spending plan for daycare and medical costs. Also available are numerous supplemental insurance packages. 401(k) or ROTH retirement with up to 5% employer matching after one year of employment. Eleven days of paid holidays and Personal time off (PTO) accruing at 130 hours annually with increases after three years of service. #J-18808-Ljbffr
