Care CoordinatorThe Care Coordinator works to improve patient outcomes and satisfaction by coordinating and facilitating patient care across a multidisciplinary care continuum while ensuring compliance with associated requirements. Coordinates assessment, planning, and implementation of health care and related services to assigned patients.
