Title:
Care Coordinator

Qualifications:

High school diploma or GED

Familiar with Developmental Disabilities Waiver service standards

Must be a licensed driver with a good driving record.


Must have a dependable insured vehicle.


Excellent computer skills including Microsoft Office with the ability to learn new applications.


Demonstrates effective verbal and written communication that is clear, well-organized, and demonstrates an understanding of audience needs.


Ability to handle confidential information and set priorities.


Must be able to pass background check and adhere to a drug-free workplace.


Licensure/Certificates:

CNA preferred but not required

CPR Certification

Job description:

Scheduling appointments/calendar management (including coordination of logistics as necessary, notices, support materials, etc.
)

Coordinates, maintains, and updates health records, reports, and confidential material.


Reviews medical records/documentation to ensure they are complete, assembles records into standard filing order, scans, uploads, and files records in the EHR.


Fax physician orders for healthcare provider(s) authorization, communicates as needed with healthcare providers to receive orders timely, responsible for all pharmacy related duties.


Assists nursing in evaluating eMARs for accuracy and communicating errors with the pharmacy, & run reports as directed to ensure compliance.
