Care Management AssistantPosition is located at Cianchette Professional Building. Work type is full time with 40 hours per week, scheduled from 8:00 AM to 4:30 PM. The Care Management Assistant assists the Regional Manager/Director and the Care Management and Community Care Team staff in providing daily duties and tasks. These include clinical review and screening of patients through the use of the EMR and claims, assignment of patients, triage of CM/CCT patient assignments per protocol, and management of new patient assignments within scope of license/certification. The Care Management Assistant provides direct patient education through telephonic engagement. This role also provides CM/CCT panel management and generates and interprets reports. The role provides patient care within license/certification scope, supports orientation of new staff, and assists with operations under the direction of direct supervisor. The role manages special projects as assigned. The role requires a strong clinical background and knowledge with extensive use of multiple modes of technology.Responsibilities include being an available resource for CM and CCT Programs, supporting new staff during orientation, problem solving and working closely with Regional Manager/Director to deploy appropriate CM/CCT resources, identifying patients in need of CM/CCT services, collecting, reviewing, screening and documenting patients' current health status and program identification data, understanding appropriate interventions that are provided and available within the CM/CCT programs, performing ongoing patient screening and planning through effective collaboration with the appropriate clinician/leader, providing telephonic follow up, management, resource identification and support of patients per protocol and policy, organizing and distributing patient and program information to CM/CCT staff in an effective and timely manner, assisting with clerical duties as assigned including report management, data reports, education materials, W: drive folders, and other as assigned, providing support for activities related to CM/CCT to facilitate ongoing communications and efficient departmental operations, compiling assigned statistical data for CM/CCT teams, demonstrating the ability to develop and build strong computer skills in a variety of software products to include, but not limited to, EMR documentation and functionality, Microsoft products and other platforms, which support Beacon Health and Northern Light Health system, and for Community Care Team, completing all aspects of monthly billing, invoicing, and attestation for services.Qualifications include Medical Assistant, Certified Medical Assistant, Licensed Practical Nurse, Certified Nursing Assistant. Additionally, for a CCT CMA position can accept Medical Assistant, Certified Medical Assistant, Mental Health Rehabilitation Technician, or Licensed Social Worker. Conditional licenses in these areas are also acceptable. Must pass background check. Attention to detail and ability to be flexible in performing a variety of tasks. Demonstrated ability to complete projects and assignments within designated timelines and ability to prioritize duties as required. Remote work and/or hybrid office setting. Autonomous, independent, self-starter required.
