Care Management AssistantShift: 1stFull/Part Time: Full TimePay Range: $20.40 $30.60Location: Advocate Christ Hospital & Medical Center 4440 W 95th St Oak Lawn, IL 60453Benefits Eligible: YesHours Per Week: 40Our Commitment to You:Advocate Health offers a comprehensive suite of Total Rewards: benefits and well-being programs, competitive compensation, generous retirement offerings, programs that invest in your career development and so much more so you can live fully at and away from work, including:CompensationBase compensation within the position's pay range based on factors such as qualifications, skills, relevant experience, and/or trainingPremium pay such as shift, on call, and more based on a teammate's jobIncentive pay for select positionsOpportunity for annual increases based on performanceBenefits and morePaid Time Off programsHealth and welfare benefits such as medical, dental, vision, life, and Short- and Long-Term DisabilityFlexible Spending Accounts for eligible health care and dependent care expensesFamily benefits such as adoption assistance and paid parental leaveDefined contribution retirement plans with employer match and other financial wellness programsEducational Assistance ProgramSchedule Details/Additional Information: 0900 1730 Mon Fri with weekend and holiday rotation.Major Responsibilities:Coordination of "Important Message from Medicare" (IM) correspondence.Obtains signatures from Medicare Beneficiaries or their emergency contact within 48 hours prior to discharge as directed by the Care Manager or Social Work Staff.Mails out appropriate letters to emergency contacts when the Medicare Beneficiary is unable to take receipt of the acknowledgement or when directed by the Care Manager of Social Work Staff.Monitors all department voicemail lines and follows up appropriately.Delivers the IM letter/Code 44 letter to the Medicare Beneficiary or their emergency contact within 48 hours prior to discharge as directed by the Care Manager or Social Work Staff.Documents all activities related to the IM/Code 44 in Care Connection.Provides clerical support to the Care Managers and Social Work Staff during the appeals process for a Medicare Beneficiary.Coordination of discharge planning.Provides required documents to vendors in a timely manner, faxing appropriate information from the patient's medical record when necessary.Assists with facility bed availability and updates the Care Manager or Social Worker of status of facility as needed until patient discharge when necessary.Identifies contracted vendors for discharge relating to home health care, nursing home placements, durable medical equipment, LTACHs, Hospice etc. Makes the appropriate referrals and communicates arrangements to the Care Manager or Social Worker, performing inquiries about benefits and eligibility when necessary.Monitors all department voicemail lines including the social work voicemail and follows up appropriately.Compilation of department statistics as requested.Updates hospital systems related to discharge.Provides reports as needed as directed by the CM Leadership.Provide Clerical support to the department.Order supplies for the department.Take meeting minutes for departmental as well as ad hoc meetings when requested.Education/Experience Required:H.S. diploma or equivalent 2-3 years clerical experience in a health care settingKnowledge, Skills & Abilities Required:Ability to use Microsoft Office Ability to handle difficult calls and maintain professional conduct Good communication skillsNot applicablePhysical Requirements and Working Conditions:Prolonged sitting and data entry Positive interpersonal skills Effective communication skills Able to work under deadlines and in fast-paced environment Able to prioritize independently and respond to multiple, simultaneous requests Able to maintain patient privacy and confidentialityThis job description indicates the general nature and level of work expected of the incumbent.
