Mental Health Kokua is seeking a Case Management Team Lead to support both clients and staff in Kona. This role is for a strong behavioral health professional who can lead case managers, carry a partial caseload, and help ensure clients receive coordinated, high-quality care.

What You’ll Do
  • Supervise and support case managers
  • Maintain a partial caseload of 10 to 15 clients
  • Oversee treatment planning, assessments, and follow-up
  • Help staff manage high-risk and high-acuity cases
  • Support quality, compliance, documentation, and continuity of care
  • Coordinate with medical, psychiatric, and community providers
  • Participate in after-hours on-call rotation
What We’re Looking For
  • Strong case management experience in behavioral health or human services
  • Ability to lead staff, hold standards, and build trust
  • Sound clinical judgment and clear documentation skills
  • Ability to manage competing priorities without losing follow-through
  • Comfortable working with clients with complex psychosocial needs
Minimum Qualifications
  • Bachelor’s degree or higher in a related field
  • Required behavioral health coursework
  • Three years of relevant experience preferred
  • Computer literacy
  • Experience serving individuals with co-occurring needs preferred
Required Credentials
  • Valid Hawai‘i driver’s license
  • Safe driving record
  • Current vehicle insurance and safety inspection
  • Criminal background check
