Case Manager Mommy Me Seaford Lewes Apply
We are looking for a Case Manager to provide quality, sufficient, and appropriate case management services to our Mommy & Me program residents at two separate locations in Seaford and Lewes, Delaware. The Case Manager's primarily role is to enhance the quality of residential services to maximize satisfaction and engagement. The successful candidate will be accountable for the full patient care cycle. Case Manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the resident’s health and human services needed.
This is a full-time position, Monday-Friday day shift position with possible occasional on-call nights and/or weekends as needed. This position will require three days at one location and two days at the other location each week.
Responsibilities
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
- Handle case assignments, draft service plans, and review case progress
- Help residents achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help residents make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations and cooperate with medical team throughout the entire case management process
- Take the extra mile and interact with residents to keep track of their progress and to ensure satisfaction
- Record cases information, complete accurately all necessary forms and produce statistical reports
- Assess and address motivational and psychosocial issues
- Adhere to professional standards as outlined by protocols, rules and regulations
Requirements
- High School Diploma or GED Required
- Bachelor's Degree in Human Services, Psychology, Social Work, Counseling or related field preferred
- Minimum of three years working experience in Case Management in Human Services or related field required
- Of which at least two years should be in Behavior Health or related field
Skills
- Excellent knowledge of case management principles, healthcare management and reimbursement
- Previous experience with psychological aspects of care
- Effective communication skills
- Excellent organizational and time management skills
- Familiarity with professional and technical emerging knowledge
