Position Summary
The Case Manager / Intensive Case Manager is responsible for assessing participant needs and coordinating services to support individualized recovery goals. This role ensures implementation of person-centered treatment plans while advocating for access to clinical, medical, behavioral health, addiction recovery, and community-based services.
The Case Manager identifies service gaps, recommends program improvements, and maintains compliance with all agency, funder, and licensing requirements. A strong commitment to RHD’s values and trauma-informed, recovery-oriented practice is essential.
This is a full-time, benefits-eligible position with paid time off.
Key ResponsibilitiesTeam-Based Planning
Participate as an active member of a multidisciplinary team providing holistic, values-driven care.
Collaborate with participants, clinical staff, medical providers, and natural supports to develop and review individualized recovery plans.
Monitor progress toward goals and adjust service strategies as needed.
Build and maintain partnerships with community agencies to expand participant access to resources.
Provide community-based case management services.
Support participants with resource linkage, life skills development, education, and community integration.
Assist participants in navigating behavioral health, addiction recovery, and social service systems.
Transport participants to appointments as needed.
Complete required assessments, service notes, care plans, and reports in accordance with agency standards and regulatory requirements.
Maintain accurate, timely documentation to ensure compliance with funder, licensing, and accreditation entities.
Participate in audits, quality assurance activities, and corrective action processes as required.
Perform other duties as assigned.
Qualifications
Education:
Bachelor’s Degree in Human Services or related field preferred
High School Diploma required
Experience:
Minimum two (2) years of experience supporting individuals in Behavioral Health, Addiction Recovery, or IDD services required
One (1) year of case management experience preferred
Certifications & Requirements:
CADC Certification required
Valid driver’s license and reliable transportation required
Must obtain CPR and MAP certification within first 3 months of employment
Intermediate computer proficiency
Working Conditions
Primarily community-based work within the program’s service delivery area.
Regular home visits and navigation of community environments required.
Must be comfortable providing services in participants’ homes.
Required to transport participants as part of service delivery.
Physical Requirements
Medium work level:
Up to 50 lbs occasionally
Up to 20 lbs frequently
Up to 10 lbs constantly
