RiseWell Community Services is seeking a full-time Case Manager to join the RiseWell's Case Management team at the affordable housing development in Far Rockaway. NY.

In this role, you will provide case management services, housing support, coordination of services to individuals in recovery from mental illness and/or substance abuse in a mixed-used housing setting with the goal of keeping the individuals independent and stably housed in on-site apartments.

Qualifications:
BA in Human Services preferred or a High School Diploma and 3 years’ experience in mental health . Relevant experience working with individuals who are homeless, in recovery from mental illness and/or substance abuse. Clean valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
