SUMMARY
Family Service League is seeking a full-time Case Manager for the High Utilization Referral program. The Case Manager provides care coordination for individuals identified in High Utilizer Referral program.

RESPONSIBILITIES
- The Case Manager will work with High Utilizer Referral Social Worker, Mobile Crisis Staff and Suffolk County Police Department- Behavioral Health Section to outreach, assessment, and link individuals disproportionally engaging with law enforcement
- Conference cases, recommendations and barriers to care with clinical staff.
- Track and coordinate outreach efforts, linkages and treatment compliance.
- Coordinate with other agency departments and community resources such as providers, housing agencies, DSS and Suffolk County Police.
- The Case Manager will engage families and other natural supports to identify individual’s needs, facilitate linkages and promote follow through.
- Utilize crisis de-escalation, motivational interviewing and harm reduction techniques to engage individuals in treatment and safety planning.
- Work as part of a multidisciplinary team, partnering with social work for in community outreach and services.
- The Case Manager will maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
- Maintain knowledge on available community resources, including but not limited to, medical, behavioral health, housing, DSS entitlements, transportation, vocational and other services that address concrete needs.
- Complete trainings and maintain proficiency with de-escalation and crisis intervention techniques.
- Abide by FSL’s Code of Ethics.
- All other duties as assigned.
QUALIFICATIONS
Bachelor’s degree required or Master’s degree in a related behavioral health field preferred (social work, mental health counseling, psychology, health administration or similar degree.)
A minimum of 1-year related experience providing care coordination in a behavioral health and/or forensic setting and demonstrated experience in interdisciplinary care coordination required.
Familiarity with NYS Entitlements, Housing, Health, and/or Mental Health/Substance Abuse Service Delivery System required; knowledge of Medicaid, Social Security and other entitlements preferred.
Computer proficiency, including Microsoft office and Electronic Health records required.
Excellent interpersonal and verbal and written communication skills required.
Strong organizational and multi-tasking skills.
Valid, clean New York State Driver’s License required.
PHYSICAL REQUIREMENTS
Ability to bend, stretch, and stand for long periods of time.
