Summary Under the direction of the Supervisor Transition of Care, provides clinically based case management to support delivery of effective and efficient patient care. This position partners with the Case Management and multidisciplinary teams to identify appropriate utilization of resources and to ensure reimbursement. EXPERIENCE Minimum 2-3 years' experience in Case Management or related healthcare field. SPECIAL SKILLS Excellent communication, interpersonal, organizational and presentation skills. OTHER Computer skills.
