Role Description This is a part-time hybrid role for a Case Manager based in Franklin, VA, with flexibility for some work-from-home arrangements. The Case Manager will be responsible for assessing client needs, coordinating care plans, and providing ongoing support to individuals with brain injuries and their families. Daily tasks include liaising with healthcare providers, tracking client progress, and connecting clients with resources that support their rehabilitation and community reintegration. The role involves working closely with clients to empower them in achieving their goals and overcoming challenges. Qualifications Case Management, Client Support, and Advocacy skills Experience in Assessment, Planning, and Resource Coordination Strong Communication, Interpersonal, and Organizational skills Knowledge of community resources and rehabilitation services for individuals with brain injuries Ability to work both independently and collaboratively in a hybrid work mode Bachelor's degree in Social Work, Human Services, or a related field (Master's degree preferred) Prior experience in a healthcare, rehabilitation, or social services setting is a plus Commitment to supporting individuals with brain injuries and improving their overall well-being #J-18808-Ljbffr
