Case Manager – Hickory House at Knox

Job Type: Full Time
About the Role:
- Under the immediate supervision of the Clinical Manager, the Case Manager is responsible for each component of the client’s treatment planning, client’s behavioral health plan, including all programmatic implementation.
- Case managers will also coordinate aftercare plans such as MAT service appointments, mental health continuums, primary physician appointments, relapse prevention support within the community including NA/AA meetings and identify transportation for client at time of discharge. The Case Managers will also meet with their assigned case load individually every 7 days to adhere to documentation requirements through Medicaid standards.

Job Qualifications:
- High School diploma preferred,
- Minimum of 1-2 years of related experience or training
- Ability to read, write, and comprehend the English language- Simple instructions, short correspondence, & memos
- Positive demeanor and clear understanding of professional boundaries with clients
- Ability to follow directives explicitly, adhere to facility policies, provide patient-driven customer service, and exercise time management skills.
- Dependability, punctuality, accuracy, attention to detail, & thoroughness are all essential to this role.
- Completion of CPR and CPI before first shift on the floor
- Schedule flexibility, holiday, and weekend rotation is REQUIRED.
- Must hold a valid driver's license with reliable transportation.
Key Responsibilities:
- Maintains adherence to HIPAA compliance in ALL communication regarding clients.
- Discharge Planning

- SNAP, BAM Documentation
- Update BHP Weekly
- Weekly Progress Notes
- Develop Relapse Prevention Plans
- Admissions/Intake Paperwork
- Compliance with EMR usage and documentation.
- Documents sessions with all clients
- Participates in monthly staff trainings.
- Collaboration with Primary Therapist
- Cultural sensitivity and ethical treatment of all clients.
- Professional boundaries, appearance, demeanor, and effective communication.
Compensation: $17-$20/hr
