Job Title: Case Manager Transitional Shelter (Families & Children)
Location: New Rochelle, NY
Salary:$55,000 - $60,000 per year
Job Type: Full-Time, On-Site
Reports to: Director of Social Services / Program Director

Position Summary

We are seeking a compassionate, organized, and mission-driven Case Manager to support families with children residing in our transitional shelter in New Rochelle, NY. The Case Manager plays a key role in helping families achieve housing stability, access critical services, and work toward long-term independence.

Key Responsibilities Qualifications What We Offer
    • Manage a caseload of up to 25 families, providing comprehensive case management services

    • Develop, implement, and monitor individualized service plans (ISPs) in collaboration with clients

    • Conduct intake assessments and ongoing needs assessments

    • Provide referrals and connections to housing resources, public benefits, employment, childcare, education, and healthcare services

    • Advocate on behalf of clients with landlords, schools, service providers, and government agencies

    • Maintain timely and accurate documentation in electronic case files in accordance with funder and agency requirements

    • Participate in interdisciplinary team meetings, case conferences, and staff trainings

    • Provide crisis intervention and support during housing emergencies, domestic violence incidents, or mental health challenges

    • Monitor client progress and adjust service plans accordingly

    • Assist families in preparing for independent living, including budgeting and securing permanent housing

    • Bachelor's Degree in Social Work, Human Services, Psychology, or related field (Master's Degree a plus)

    • At least 1 2 years of case management or direct service experience with vulnerable populations, preferably families or children in shelter/housing settings

    • Strong understanding of trauma-informed care, housing instability, and the NYC/Westchester human services landscape

    • Excellent organizational, time-management, and communication skills

    • Proficiency with case management software and Microsoft Office

    • Bilingual (English/Spanish or other relevant language) a plus

    • Ability to work flexible hours, including some evenings or weekends as needed

    • $55,000 - 60,000 annual salary
