Rate: $54,000 $56,400 per year
Type: Permanent, Full-Time
Shift: Monday Friday | 9:00 AM 5:00 PM EST
Work Arrangement: Hybrid (2 days remote, 3 days onsite; no home visits required)

Position Summary:

The Case Manager will assess client needs, determine eligibility for in-home services and public benefits, and manage ongoing case work and care plans. This role reports directly to the Social Work Supervisor and supports clients through advocacy, referral, and continuous care coordination.

Key Responsibilities:
  • Conduct intake assessments and screenings as needed.

  • Provide accurate information and assist clients in accessing entitlements, benefits, and services.

  • Apply and recertify clients for public benefits.

  • Coordinate and arrange for appropriate service delivery.

  • Use respectful and empathetic communication when interacting with clients and their families.

  • Maintain complete and up-to-date case notes and documentation using the program's data system.

  • Monitor client progress, reassessments, and service effectiveness.

  • Prepare reports, maintain records, and compile relevant statistics.

  • Participate actively in supervision, team meetings, and training.

  • Assess clients' social, emotional, and environmental needs (in-office only).

  • Develop and implement individualized care plans.

  • Provide ongoing support and follow-up contact with clients.

  • Offer supportive counseling as necessary.

  • Supervise social work students when assigned.

  • Coordinate translation services for clients as needed.

  • Perform additional duties as required by the program or supervisor.

  • Maintain excellent attendance and adhere to professional appearance standards.

  • Support the organization's mission through professional conduct and teamwork.

Qualifications:
  • Education: Bachelor's degree required.

  • Language: Fluency in Korean and English is required.

  • Experience: Minimum of one year in senior case management or related field.

  • Proficient in Microsoft Office Suite and other digital tools.

  • Strong communication, active listening, and customer service skills.

  • Ability to work independently and manage multiple priorities with attention to detail.

Working Conditions:
  • Office-based environment with regular computer and phone use.

  • Hybrid schedule: 2 days remote, 3 days onsite at the Flushing location.

  • No travel or in-home visits required.
