Overview
We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) Case Manager to join our care team in Jeannette, Pennsylvania. This full-time position offers competitive pay, comprehensive benefits, and a supportive, team-oriented environment. The RN Case Manager will lead and collaborate with an interdisciplinary team to manage and deliver personalized patient care plans for individuals receiving care at home.

Key Responsibilities

• Provide leadership in the management and delivery of patient care plans

• Support patients in Irwin and surrounding areas

• Collaborate with the care team to ensure continuity and quality of care

• Meet critical charting and documentation deadlines

• Effectively self-manage time and daily schedule

• Utilize EMR systems, tablets, and mobile technology for charting and communication

Schedule and Shift Details

• Monday through Friday, 8-hour days

• One evening of on-call per week (5:00 p.m. 10:00 p.m.)

• One weekend per month (Saturday/Sunday)

Qualifications

Required

• Associate degree or higher in Nursing

• Current Registered Nurse (RN) license in Pennsylvania

• Minimum of 1 year of clinical nursing experience

• Proficiency with Electronic Medical Record (EMR) systems such as TruChart EHR or similar

• Valid driver's license and reliable transportation

• Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications from the American Heart Association

• Willingness to travel within the service area as required

Preferred

• Previous experience in home health or case management

• RN license number provided upon application
