Program Type: Intensive Outpatient (IOP) & Partial Hospitalization Program (PHP)

Were looking for a compassionate, organized, and motivated Case Manager to join our IOP/PHP treatment team. This role is ideal for someone who thrives in a fast-paced clinical environment and is passionate about supporting clients through every step of their recovery journey.

Key Responsibilities

• Coordinate care for clients enrolled in IOP and PHP services
• Conduct biopsychosocial assessments and assist with treatment planning
• Provide ongoing case management, referrals, and resource coordination
• Collaborate closely with therapists, psychiatrists, nursing staff, and external providers
• Support discharge planning and continuity of care
• Maintain accurate, timely clinical documentation in accordance with program and regulatory standards
• Advocate for clients and support engagement in treatment

Qualifications

• Bachelors degree in Social Work, Psychology, Counseling, or related field (Masters preferred)
• Case management experience in behavioral health, mental health, or substance use treatment strongly preferred
• RAD-T, CADC or SUDCC required
• Familiarity with IOP/PHP levels of care
• Strong organizational, communication, and time-management skills
• Ability to work collaboratively as part of a multidisciplinary team
• Experience with electronic health records (EHR) a plus
