Cornerstone Hospice & Palliative Care is a non-profit provider of end-of-life and palliative care services, dedicated to supporting individuals with life-limiting illnesses. Established in 1984, the organization is committed to delivering compassionate care to its patients, their families, and the community. Serving nearly 1,200 patients daily, Cornerstone operates across seven Central Florida counties, including Lake, Orange, and Osceola, with the support of six inpatient hospice facilities. With a strong focus on dignity and quality of life, Cornerstone is a trusted partner in compassionate care. Role Description This is a full-time on-site role for a Case Manager, located in The Villages, FL. The Case Manager will deliver patient-centered care by assessing and monitoring individuals in hospice or palliative care. Responsibilities include developing and implementing care plans, providing education to patients and their families, coordinating care across interdisciplinary teams, and ensuring compliance with care protocols. The role also involves documentation, communication with healthcare providers and families, and advocating for patients' needs. Qualifications Strong clinical skills, including patient assessment, care planning, and care coordination Excellent communication, compassion, and advocacy abilities to support patients and families Time management, problem-solving, and organizational skills for managing caseloads effectively Knowledge of hospice and palliative care policies, procedures, and compliance standards A valid Registered Nurse (RN) license in the state of Florida or relevant case management credentials Proficiency with electronic medical records and documentation systems Experience in end-of-life care or similar settings is preferred Reliable transportation and ability to travel within assigned service areas #J-18808-Ljbffr
