Responsibilities:
1. Develop and maintain rapport with individuals referred to CSP.
2. Provide telephone outreach and home/community visits if needed
3. Provide hands-on case management support to clients, including providing short-term transportation to support engagement with providers and social services
4. Link individuals to resources and valuable information to improve health and wellness.
5. Help advocate for individuals to improve their access to care.
6. Complete required written documentation per program expectations.
7. Document in HHMIS System
8. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
License/Certification/Education:
Required:
1. Must have a current valid US-issued driver's license and ability to provide registered, inspected and insured automobile for work related purposes including transporting clients in your own vehicle.
2. All candidates will be subject to a CORI review as outlined in the regulations set by the Massachusetts Executive Office of Health and Human Services.
Experience/Skills:
Required:
1. Strong communication and listening skills.
2. Comfortable working independently and within a group.
3. Flexible yet organized.
Preferred:
1. Bilingual (Spanish/English or Vietnamese/English) a plus.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
