Job Summary:
We are seeking a highly skilled Case Worker/Case Manager to join our team and will play a vital role in providing essential support services to individuals in need. Will be conduct comprehensive assessments, guide families through the application process, and connect them with appropriate mental health services. Dedication and expertise will contribute to positive outcomes and improve the lives of those we serve.
Key Responsibilities:
  • Conduct face-to-face CNAS assessments with youth and families.
  • Manage incoming referrals, ensuring initial contact within 48 business hours.
  • Guide families through the application and documentation process for CMHW eligibility.
  • Build and maintain relationships with mental health service providers for seamless transitions.
  • Participate in training and development sessions to stay updated on best practices.
  • Track, document, and report program data via the Tobi database system.
  • Travel within assigned counties for assessments and follow-ups.
Education and Requirements:
  • Bachelor's degree in Social Work, Psychology, Counseling, Nursing, or a related field with 2+ years of clinical experience; OR
  • Master's degree in Social Work, Psychology, Counseling, Nursing, or a related field with 2+ years of clinical experience.
  • Valid Indiana driver's license and current vehicle insurance.
  • Clear Department of Child Services (DCS) background checks.
Required Skills:
  • Exceptional time management to meet 15-business-day referral processing deadlines.
  • Strong interpersonal skills for engaging with diverse families and providers.
  • Proficiency in documenting and navigating database systems like Tobi.
  • Commitment to upholding HIPAA and other confidentiality standards.
