JOB DESCRIPTION:
The Team Lead CDCA, Case Management oversees a team of CDCA/Case Managers providing services to individuals in recovery from substance use and co-occurring disorders. This position supports the day-to-day coordination of case management duties and provides leadership, training, and supervision to ensure quality and compliance. The Team Lead will carry a caseload and assist in providing direct client support while serving as a liaison between staff and leadership to promote efficient service delivery. They will perform treatment planning, assessment, crisis intervention, individual and group counseling, case management, and education services as they relate to abuse of or dependency on alcohol and other drugs under the Program Director or designee. They will serve as an advocate and liaison for both the CDCA, Case Managers and the clients being served. They assist in helping navigate treatment and support services, acting as an advocate and coordinating care.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Oversee a team of CDCA, case managers, provide guidance and support, mentor new staff, assist with onboarding, and help with training.
- Collaborate closely with the Program Director, Managers and staff support the day-to-day operational needs of the facility.
- Review and audit documentation for accuracy, completeness, and compliance with policies and state/federal regulations.
- Coordinate with treatment team to ensure continuity of care. Advocate for clients and collaborate with community resources, such as housing, employment, and healthcare services.
- Assist with monitoring caseload distribution, productivity, group assignments and staff adherence guidelines. Monitors and assists with programing, ensuring group and individual therapy guidelines are followed.
- Assists with coordination and attend multidisciplinary team meetings; advocate for client needs and ensure case continuity.
- Maintain a caseload and provide CDCA, case management services including individual counseling, referrals, linkage to community resources, advocacy, and individualized support planning.
- Participate in internal quality improvement and performance monitoring processes.
- Collaborate with external providers and community partners to enhance client services.
- Support crisis response efforts and problem-solve barriers to client progress.
- Orients patients to the treatment programs operation and procedures.
- Conducts alcohol and drug abuse counseling sessions on a regular basis per state regulation, or as part of the individual treatment plan.
- Conduct weekly assessment of individual client needs and assist with a plan to address those needs.
- Conducts intake interviews with clients to gather information about their substance abuse history, mental health status, and other relevant factors.
- Based on the information gathered during assessments, they will help to develop treatment plans with clients that will address their unique needs and goals.
- Develops within (10) days of entry or as required by state guidelines, whichever is more stringent, a formal treatment plan documenting patient goals and update utilizing the same guidelines.
- Implements or revises patient treatment plans with the patient as required per state regulation and record a review of the patients progress.
- Monitor clients' progress throughout their treatment and adjust their treatment plans as needed.
- Provide counseling services to clients, either one-on-one or in group settings. They may help clients to identify triggers for substance use, develop coping skills, and work through emotional issues related to their addiction.
- Assist patients in identifying, maximizing, and relating their strengths to appropriate social, educational (academic and vocational), and occupational goals.
- Helps patients prepare and adjust to a drug free existence, and to offer assistance (emotional) in problematic issues.
- Maintains patient records that are complete, comprehensive and well organized. Assure documentation is completed per OMHAS, regulatory, and agency requirements following best practices.
- Attend required meetings, including individual and group supervision, treatment team meetings, training, and community events as appropriate. Participate in at least one hour of supervision per week.
- Evaluate, facilitate and monitor services and clients progress in relationship to established goals and objectives.
- Work to obtain aftercare placements such as sober living, partial hospitalization, or outpatient placement for aftercare services.
- Facilitates client benefits (e.g., SSI, Medicaid, SSB, Medicare).
- Holds a basic understanding of alcohol/drug abuse and addiction. Additionally, keep updated on current drug abuse and treatment information, especially current trends in Opioid Maintenance Treatment.
- Ensure compliance with the Ohio state regulations and ethical guidelines for CDCA practice.
- Other duties may be assigned.
QUALIFICATIONS:
- Must have High School Diploma or GED.
- CDCA certification or higher through the Ohio CDP Board is required. LCDC III preferred.
- Minimum of 2 years of experience in case management, Substance Abuse Disorder or Recovery Support Services.
- An associates degree in human services, Chemical Dependency, or another related field preferred.
- CPR/BLS Certification required.
REQUIRED SKILLS OR ABILITIES:
Required Skills for this position include but are not limited to:
- Must demonstrate a current knowledge of counseling and research findings and literature pertaining to alcohol/drug abuse, domestic violence, sexual assault, child abuse/neglect, trauma informed care and mental health issues.
- Ability to interact with a multi-disciplinary team to assist in facets of the recovery program relating to physical health.
- Have the ability to communicate effectively with all members of the treatment team to ensure the safety of the client.
- Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which is related to work as a member of an addiction treatment team.

- Computer literate with experience in a Windows environment.

This job operates in an inpatient facility environment at an OVP Health location that is open 24/7.
