Phoenix Home Care and Hospice in St. Louis, MO is looking for a Consumer Directed Services / In Home Services Care Coordinator!

Job Duties:

•     Oversight and compliance of program requirements 

•     Interacting with Consumers/Clients and Attendants/Caregivers 

•     Coordinating Consumer/Client and Attendant/Caregiver On-boarding 

•     Opening new Consumer/Client cases in their home 

•     Troubleshooting problems and providing resolution 

•     Ability to handle a multi-line phone system 

•     Highly Organized & Detail Oriented 

•     Annual in-person visits to Consumers 

•     Conduct monthly case management calls via phone 

•     Payroll processing related duties 

Experience Needed:

•     Past experience in a medical field preferred 

•     Previous Care Coordination, In Home, or CDS experience is a plus 

•     Medical office or healthcare experience preferred 

•     Prior management experience preferred.
