What You’ll Do Appropriately represent at the Board of Directors level. Oversight for Integrated Wellness and Care Coordination Oversight for Medical, Dental and Behavioral Health, Community Outreach, and Information Technology throughout the organization. Oversight for clinical leadership and joint oversight with President/CEO for provider leadership. Oversight of corporate annual goals, including quality clinical goals, patient access and patient experience. Works in collaboration with Chief Financial Officer and Chief of Talent Development. Develop and foster effective collaboration between clinical departments, divisions, medical, dental, behavioral health services leadership, and other affiliated services (inside and outside of CWH) to ensure an integrated wellness and care coordination/management approach to providing services and fulfilling CommWell Health’s clinical and educational goals and responsibilities. Ensures Population Health Management services which support the Patient Centered Health Home model including Care Coordinators/Case Managers, the use of technology for care coordination, patient education to support self-management and care decision making and achievement of completion of health reminders yielding improved patient outcomes and improved clinical performance metrics. Facilitate a highly matrixes approach in the development of CWH services and display an ability to work effectively within the center’s decision making and organizational structure. Oversees Senior Director of Nursing and workforce and resource decisions for clinical areas of CWH. Oversees Education Manager and educational activities as well as clinical competency development and management. Where appropriate, represent CWH to the external market, as well as internally through the application of community relations and marketing activities. Work through the management team and medical leadership of CWH, serving as a resource to help reduce costs, enhance revenues, achieve effective utilization and quality goals and objectives, analyze and utilize information to develop and support management decisions. Communicate key information to the stakeholders of these service areas with respect to managed care, marketplace needs, the competitive environment, cost management, and customer focused services. Expand CWH outreach activities and referral networks to ensure effective partnerships are formed which will facilitate the development of a comprehensive and geographically dispersed integrated health care system. Lead and support key committees pertaining to Care Coordination and Quality. Perform other related duties incidental for the work described herein as may be assigned or delegated. This role requires significant leadership skill and ability. This position will be accountable for the overall success of clinical operations of CWH, Quality Indicators and Core Measures. Attention is to be given to systems, program development, quality, fiscal management, compliance and clinical management measures, physician relationships, outreach strategies, work culture enhancement and internal communication and consensus-building. Ensure that CWH’s resources are focused enterprise-wide on optimally satisfying the health care needs of those we serve. Overall assessment of multispecialty operations and integration of services with medical, dental, behavioral health, specialty referrals, as well as, other internal and external services needed for disease and case management. Ensures JCAHO and HRSA readiness, compliance, and directs and oversees attainment of goals and objectives. What We’re Looking For Required: Bachelor's degree (or actively pursuing) in health-related program Minimum Ten (10) years’ experience in healthcare management. Medical/Clinical background is a plus. Current NC RN license BLS Certificate from American Red Cross or American Heart Association Clean driving record with current NC driver’s license Preferred: Master’s Degree (or actively pursuing) in health-related field Ten (10) years of progressively responsible experience in a community health center environment Why Join CommWell Health?
