We're looking for a skilled Care Coordinating Manager to be the operational heart of our client and caregiver experience — someone who brings structure, warmth, and precision to a fast-paced environment where every detail directly affects patient outcomes.Client & Care CoordinationServe as the primary point of contact for clients, families, and referral sourcesConduct intake calls, ensure accuracy of all intakes and referrals, and oversee smooth service setupCoordinate care schedules aligned with client service plans and clinical needsAttend client assessments with the nursing team as neededMaintain ongoing follow-up to ensure client satisfaction and resolve service or scheduling issues promptlyCoordinate with hospitals, facilities, and care managers to facilitate smooth transitions from inpatient settings to home-based careEnsure all discharge documentation, authorizations, and service orders are in place prior to the patient's return homeCommunicate discharge plans to the clinical team and scheduling staff to ensure immediate coverage upon arrivalServe as the point of contact for families navigating the discharge-to-home process, providing guidance and reassuranceFollow up post-discharge to confirm services are active, appropriate, and meeting the patient's evolving needsScheduling team leadershipDirectly manage and oversee the scheduling team, providing day-to-day direction, support, and accountabilitySet performance expectations for scheduling coordinators and conduct regular check-ins and feedbackEnsure scheduling team adheres to agency protocols, fill-rate targets, and client coverage standardsServe as the escalation point for complex scheduling conflicts or coverage challengesMatch caregivers to clients based on skills, availability, and compatibilityMaintain accurate scheduling records and ensure all shifts are staffedNurse incentive program managementOversee and administer the nurse incentive program, including shift bonuses, referral rewards, and retention incentivesTrack incentive eligibility, usage, and outcomes to assess program effectivenessCollaborate with leadership to design, adjust, and communicate incentive structures that drive fill rates and caregiver engagementEnsure incentives are applied consistently, fairly, and in alignment with agency budget and policyCaregiver recruitment & onboardingLead caregiver recruiting activities: posting listings, screening candidates, scheduling interviews, and managing follow-upsConduct onboarding presentations for new hires and ensure understanding of agency policies and proceduresComplete I-9 verification and maintain organized, audit-ready employee filesVerify credentials, certifications, and all required compliance documentationSupport caregiver engagement, appreciation, and retention effortsCompliance & administrationMaintain accurate client and employee files that meet regulatory standardsEnsure all documentation is properly completed, stored, and audit-readyCollaborate with leadership to continuously improve processes and service qualitySupport day-to-day office operations so leadership can focus on growth and business developmentRepresent the agency at community outreach events and referral activities as neededQualificationsRequiredExperience in home health care, healthcare staffing, or care coordinationKnowledge of LHCSA operations, compliance, and caregiver credentialingDemonstrated ability to supervise or lead a teamStrong organizational and multitasking skillsExcellent written and verbal communicationProficiency with scheduling software and Microsoft Office (Excel, Word)Valid driver's license and reliable transportationPreferredPrior experience in a Licensed Home Care Services Agency (LHCSA)Experience supporting hospital discharge planning or transitions of careExperience managing incentive, bonus, or recognition programsFamiliarity with Medicaid and managed care payer requirementsDemonstrated ability to manage high-volume scheduling environmentsCompensation & Benefits$95,000/year401KMedical, Dental, and Vision InsuranceEAPReferral Program
