Job description
Position Title: Clinical - LTSS Service Care Manager
Location: Corpus Christi, TX Travel Required in local area. (Assessments will take place via home visits in Nueces and surrounding areas)
Schedule: 8:00 AM 5:00 PM
Duration: 3 Months (Possible Extensions)
Walk me through the day-to-day responsibilities of this role and a description of the project (Outside of Workday JD):
  • SC will have 3 days of visits and 2 days of documentation weekly
  • SC will have a caseload and work assigned tasks
  • SC will complete H2060 assessment
  • SC will fill put required forms for each assessment
  • SC will make follow up calls
Describe the performance expectations/metrics for this individual and their team:
  • SC will complete documentation within 3-day turnaround time
  • SC will stay organized and ensure work they do not fall behind
  • SC will maintain a 92% or high in Audit scores
Tell me about what their first day looks like:
  • Get log in information
  • Communicate with IT for equipment needs
  • Complete any additional forms for processing
