Clinical Ltss Service Care Manager Apply
Title: LTSS Service Care Manager
Duration: 6 months
Shift: 8:00am-5:00pm, Monday to Friday
Location: South Texas - Edinburg
Day-to-Day Responsibilities:
- Assist in telehealth biannual assessments and conduct in-person assessments as needed if in the area.
- Complete assigned tasks, document findings, and ensure accurate record-keeping.
- Coordinate with multi-disciplinary teams to support member care.
- Conduct follow-up calls to members as part of case management duties.
- Participate in any other projects as they arise.
Performance Expectations/ Metrics:
- Must meet strict turnaround times for documentation of assigned assessments.
- Address tasks promptly and ensure responsiveness.
- Show flexibility and maintain professionalism in interactions.
First Day Overview:
- Meet with the Team Lead and Manager for orientation and initial training.
- Shadow other Service Coordinators to learn workflows and best practices.
Preferred Background/ Previous Roles:
- Licensed Vocational Nurse (LVN) with experience in case management.
Required Skills/Experience:
- Active Licensed Vocational Nurse (LVN) license
- Experience in case management
