Base pay range $23.62/hr - $49.29/hr Responsibilities The Social Worker evaluates the mental, emotional and physical status of patients while managing care coordination needs to develop and execute a discharge plan. The Social Worker creates a discharge plan based on an initial assessment and conducts ongoing reassessments with modifications as required, communicating with doctors, nurses, home health companies and other healthcare personnel and agencies.
