Job Opportunity: CNA LTC/SNF in Breckenridge, MN
- Position: CNA LTC/SNF
- Location: Breckenridge, MN
- Pay Rate: $28/hr
- Shift: Day/Night- 8 or 12 hours shift
- Shift Notes: D/N Rotate - EOW, 6p-6a or 2p-1030p, every other holiday
- Work Week: 36 hours per week
- Duration: 13 weeks
- Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?): 8 or 12hr shifts - 6a-6:30p, 6p-6:30a, 6a-2:30p, 2p-10:30p, 10p-6:30a // cycle scheduling
- Radius Rule: 50 miles AND candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity.
Your Key Responsibilities:
Responsibilities:
- Provide excellent patient care, including assistance with daily activities and personal hygiene.
- Respond to patient calls, assess needs, and provide appropriate assistance.
- Monitor and record vital signs, food intake, and urinary/fecal output.
- Gather information from medical professionals about patient conditions and treatment plans.
- Remind patients to take medications and supplements. Stay updated on CNA training and adhere to facility policies and procedures
What We're Looking For:
- Unit Notes
- Department: LTC CNA
- *Certifications Required: BLS*
- MN CNA License required at time of submission.
- NO CLINICAL INVOLVEMENT
- # of Beds/Residents: 40
- EMR: PointClickCare
- Ratios: RNs: Days: 1:20, Nights: 1:40, CNAs: 1:10
- Common diagnosis / Types of patients: CHF, COPD, Alzheimers, Parkinsons, Diabetes
- Are there IVs or titratable drips?: Minimal IV meds - DON/Charge Nurse/House Supervisor: Charge Nurse
- Weekend Requirements: Every other weekend
- Holiday Requirements: Every other holiday
- Scrub Color: Any color or pattern scrubs
- How many shifts of unit/department specific orientation?: 2-4 days
- Additional Notes: Has a home like work environment, strong team work, and strong work ethics
- RTO needed upfront (7 or less days per 13 week contract).
- Additional RTO will not be approved, including after submissions.

Ability to Relocate:
- Breckenridge, MN Relocate before starting work (Required)
Work Location: In person
Requirements:
- CNA MN license
- BSN Required: NO
- Experience in LTC/SNF
- Basic Life Support (BLS) certification.
- Good communication skills.
- COVID Vaccination
