Summary: The Community Care Manager plays a key role in coordinating care and managing high-risk members in a community-based setting. This position involves conducting assessments, developing care plans, and providing clinical support in collaboration with medical teams. The role also includes leadership responsibilities, such as supervising Community Care Connectors and assisting with program operations.

Position Info:

  • Title: Community Care Manager
  • Client: AmeriHealth Caritas
  • Duration:3 Months
  • Location: 11631 Kew Gardens Avenue, Palm Beach Gardens FL 33410
  • Work Mode:Hybrid

Responsibilities:

  • Identify members for care coordination and case management services in a community setting
  • Conduct in-home environmental and physical assessments for high-risk members
  • Collaborate with medical management and community health teams to establish care plans
  • Provide basic clinical care within the scope of practice in collaboration with medical teams
  • Supervise Community Care Connectors in delivering care and education on chronic disease management
  • Assist with operational responsibilities and serve as a subject matter expert as needed

Skills/Qualifications:

  • Minimum of three years of experience conducting health assessments and delivering care in community settings
  • Experience leading teams in a matrixed environment
  • Proficiency in MS Office, electronic medical records (EMRs), and documentation programs
  • Ability to travel extensively; valid driver's license and automobile insurance required
  • Must reside in Palm Beach or St. Lucie Counties, Florida, and conduct monthly in-home visits
  • State of Florida Registered Nurse (RN) with at least two years of pediatric experience with medically complex children, or Master's Degree in Social Work (MSW) and Licensed Clinical Social Worker (LCSW) in Florida with at least one year of related experience
