Description

Hennepin County Public Health is seeking a Senior Community Health Worker with a preference in an applicant who is fluent in Spanish in addition to English to work with its Tuberculosis (TB) Outreach Program. This position will work with people with active TB in the community, and will include assisting patients with barriers to care and serving as an advocate. This position will also provide daily home visits (Monday through Friday) to a caseload of patients to directly observe the ingestion of their tuberculosis medications.

Location and hours:
This position is onsite at the Public Health Clinic at 525 Portland Avenue South, Minneapolis, MN 55415 and will require traveling throughout Hennepin County to visit clients in their homes. Work hours for this position are Monday through Friday, from 8 a.m. 4:30 p.m.

In this position, you will:
  • Identify and assist patients in overcoming barriers that may prevent them from adhering to TB treatment.
  • Observe patients ingest their TB medications.
  • Report to and follow directions of the TB Services nurse supervisor and nurse case managers.
  • Ensure that patients adhere to TB medical orders, TB clinics, and other related medical appointments.
  • Assist with locating patients in the community who are suspected of having TB.
  • Educate patients about prevention and treatment of tuberculosis.
  • Document interactions with patients in an electronic medical record such as EPIC.
Need to have:
  • One of the following:
    • Completion of two years or more of college in social/behavioral sciences or another field related to the area.
    • Two years or more of experience working in a community outreach or public health setting.
  • A valid driver's license, access to reliable transportation to travel to client's homes within Hennepin County, and the ability to obtain a Hennepin County driver's permit.
Nice to have:
  • Ability to communicate fluently in Spanish, in addition to English, both verbally and in writing.
  • Experience:
    • Performing face-to-face outreach within the community.
    • Assisting families in problem-solving to address barriers to accessing healthcare.
  • Knowledge of:
    • Local public health and/or community resources and programs serving families on Medical Assistance.
    • Health care systems.
    • TB and other infectious diseases.
  • Ability to:
    • Establish and maintain effective working relationships with staff, community agencies, and culturally/socioeconomically diverse clients.
    • Communicate information to people with different levels of understanding and diverse backgrounds.
Candidates with fluency in other languages in addition to English are encouraged to apply. Bilingual candidates who pass a language assessment may qualify for additional bilingual pay and duties.
